1. Please click on the Open Enrollment Button Below

2. The Open Enrollment Form will download to your Download Folder.

3. Locate the downloaded file and Open the file in Microsoft Office (Please enable macros).

4. Fill out the necessary fields

5. Click submit

6. This will open a dialog box to your Outlook Mail.

7. Please send this email.

8. If you do not have Outlook, please use File - Save As to save the file and email the form to Andrew Virtual Academy at andrewvirtualacademy@andrew.k12.ia.us.
